TCU Mental & Physical Health and Trauma Assessment Form

The TCU Mental and Physical Health (HLTHForm) and Trauma Form (TRMAForm) provide a structured framework for screening individuals for psychological distress, physical health issues, and trauma. This resource includes the 10-item Kessler psychological distress scale (K10) to evaluate levels of distress Furthermore, it features a comprehensive 17-item trauma assessment based on the National Center for PTSD guidelines to help identify disturbing memories, avoidance behaviors, and emotional numbness linked to stressful experiences.
